Private Acupuncture (PA)

$120.00/70 minutes
A 70-minute session acupuncture treatment that may include adjunct therapies such as cupping, e-stim, moxibustion, tuina, gua sha, at the practitioner’s discretion. Treatments are done with a thorough medical intake and practitioner provides nutrition and exercise recommendations as deemed necessary.

Basic Acupuncture

$80/45 minutes
A 45-min session of acupuncture treatment only, no extra modalities are provided. Patients are encouraged to wear something comfortable and that will allow the practitioner to needle the body points.

Microneedling (Micro)

$400.00/75 minutes
The variance depth of the needle penetration offers the skin rejuvenation levels from the simple enhancement of product absorption to the clinical treatment of scars, wrinkles, acne, and pigmentation. Celluma LED light therapy is used as an adjunct to reduce any redness and inflammation.

Nanoneedling (Nano)

$250.00/60 minutes
The benefits of the serum infusion treatment include pigmentation reduction, increased cell turnover, skin rejuvenation and an immediate glow with no downtime. This treatment can be done weekly to exfoliate the skin. Celluma LED light therapy is used as an adjunct to reduce any redness and inflammation. Shown in portal

Bodywork Therapy

$60/$120for 30min/70min
Combination of cupping and massage with practitioner uses core products with variable speed massager and percussion muscle massage gun. This therapy brings fresh blood to the area and helps improve circulation. Traditional cupping, sometimes referred to as “fire cupping,” uses heat to create a vacuum-like suction inside of glass cups. In modern times, cups that use a small pump to create suction have also been introduced.

Office Policy

Insurance Billing

We are happy to bill your insurance company for you. If you would like us to bill your insurance, we will contact your insurer(s) and bill them the reasonable and customary fee for services rendered. If we are unable to obtain a verification of benefits from your insurer for any reason, we will require a discounted time of service payment at the date and time of your appointment.You are responsible for all co-payments, deductibles and other adjustments made by your insurer(s). You are responsible for and will be billed for any resulting unpaid balance.

Any cosmetic acupuncture service is cash based only, insurance is not applicable for this service.

Cash Billing

Time of Service Payments.Patients not billing insurance will be required to pay at time of service. A time of service discount is provided for these patients. For your convenience, ProviStar Acupuncture accepts cash, check, and credit card.

MissedAppointments/LateCancellations

All appointment cancellations must occur within 24-hour of the appointment. If it is less than 24 hours, you will be charged $60 for the missed private initial appointment and $45 for a missed private returns appointment. Late arrivals more than 15 minutes past your scheduled appointment time are subject to the cancellation fee. You are responsible for and will be billed the resulting cancellation fee.

Past Due Accounts

Accounts greater than 30 days past due will be charged a $10 administrative fee. Accounts greater than 90 days overdue will be sent to a collection’s agency.
